How to improve my poor communication skills?

I think I frustrate people. It's not intentional but I'm just a bad procrastinator and a bit of a dreamer. I've also been fairly depressed lately for the past few months, which maybe has something to do with it as well.

My communication skills are not great. I'm talking about life in general, not at work, where I can step up to the occasion. But when I get emails from people, I don't always respond promptly. Sometimes I let them go for a week or two, telling myself 'I'll reply to that later'. Later never seems to come sometimes.

It's obvious: I just need to do it. Reply to emails promptly. Return calls in a reasonable time frame. But I am REALLY bad at following through. How can I motivate myself to be a good communicator, all the time, every time?
