How to handle employee/employee altercation? Should HR be informed?

I have a co-worker who has been saying negative things about all coworkers behind their backs. Pertaining to me, she was ill-speaking about my competency level. I made an indirect comment infront of her and she approached me aggressively saying I should keep my comments to myself. I didnt respond. Is this grounds to consult HR or management for harassment? How would you handle it? My method of ignoring kinda takes a blow to my ego as I seem like the "punk."
