How to get Medical Certificate from the Hospital?

Hi,
My mother had GASTRECTOMY operation almost two months ago. For the past month and a half I am trying to get a Medical Certificate from the London Royal Hospital. The nurse gave me the printed sheets from the NHS comouter system and they assuring me that this is the kind of Medical Certificate they provide to the patients. When we appied for the benefits, we've send all the given papers, but they are still asking us to provide them with proper Medical Certificate.
I suppose that Medical Certificate should be written by the Surgeon or Doctor explainign in details what kind of operation they've carried out, date and everything must be in details, them stamped and signed... Also she is an EU citizen and we require this document to provide to our countries Health authorities.
Could somebody help me and explain in more details how the Medical Certificate should look like and if anybody experienced the same situation

i am not too certain just what an actual 'medical certificate" actually is, could you enlighten me there? since this is not in the US where i am from i am not too certain just what type of actual documents you need but i can tell you about the stuff here. in the US, you are actually able to get a full transcription of what took place during your surgery of any type, they are called "op notes"? every surgeon, and i would assume where you live too just has to make a transcription for legal purposes as to what took place during any surgery just so everything is down on paper. that you can obtain along with what is actually contained in whats called your 'central file' from the hospital where you had the surgery that just contains all the various documents of your stay there and what the nurses did and the docs writen orders, and just the basics stuff too.

but also obtaining the bill from that stay would also document in detailed itemized ways the surgery and the stay too. i am just not certain what you need, or for what purposes. but i have always obtained all my hospital and surgical records, including the op notes from all my six surgeries and my aneruysm coiling too with absolutely no problems. i would think even tho you live somewhere else that you would still be entitled to your own medical records. if you could explain what it is exactly what you need and why, i may be able to at least give you some level of guidance. i am just not familiar with what an actual "medical certificate" actually is. but if it is something that you are actually required to get or have to have for some reason, the surgeon or hospital would be required to just do this since it is some sort of needed/required documentation, you know what i mean? Can you contact the NHS and tell them that this is what the hospital says they need? And then ask what's different about what they are looking for and from whom you should obtain it.

Well my mother had to apply for a dissability and aslo back in Lithuania she needs to prove that she had an operation in order to get her insurance...
You know, I heard from couple of patients that they had similar issues regarding getting all the medical records, it just seems that doctors cant be bothered. But thanks again, on Monday we are having a meeting with surgeon as they have received our complaint, it seems that they are finally responding to our queries and hopefully we will be able to sort everythign out...
