How to bring back sport in your life after a long break?

Last year I started doing sport and following a diet and I lost like 10 kilos, then in summer I stopped that I I gained 5 kilos back. Now I've bought a year in gym and I'm planning to start it all over again (as of course, it is difficult for me now to do the things I could have done before the break).
But when it comes to "now i have to do to the gym" i find million reasons to stay at home. i'm not a lazy person and i do have will power, but there should be some psychological reasons (maybe I'm afraid to start it all over again or smth)...

How can I introduce sport to my life once again so that it become my constant habit?
