How to answer a phone call at work for your boss?

I seem to be lacking in this area. I help my boss out with answering his calls since he does not have an assistant at the moment. For some reason I have a terrible time taking his calls. For one, I can't remember some callers' names even if I write it down - they say it so darn fast. Is it okay to ask again?

Another dilemma is asking "someone's affiliation." My boss gets tons of junk calls and I don't know the proper way to ask them what company they work for. It just seems so weird to ask someone who they work for....I never recall someone asking me that question if I called up a receptionist desk.

Can someone please help me out with how to master taking calls? My boss is getting pissed off that I am not so good about asking for a caller's company info. Even a generic greeting will help.
