how to allow someone to make medical decisions?

Ok so here's the thing basically I work in a VERY dangerous profession where the chances of getting hurt or killed are high I live in a northern part of Canada far from family its basically me and a room mate anyway I want to give my room mate the ability to sign consent forms and so on in case i am severely hurt on the job how might I do this
