how many people die each year from bicycle accidents?

I don't know worldwide, but in the United States it's about 800 or 900 per year.

Most of the fatal accidents involve cars.
 
Not nearly as many that die in car accidents. Cycling deaths caused by cyclists falling off their bikes and no vehicles involved are very few. Deaths are caused by vehicle contact for both cyclists and pedestrians. I cannot give you any figures but I am sure you could find some under Government statistics on accidents with a Google search.
