How long before relief of pain after injection?

I just read some of your earlier threaRAB Myofascial, I realize now that the PT I went to for my neck after my accident, was performing MR on me, but calling it deep tissue massage, so I have had that for treatment. It worked very well on me. I am going to the PM boarRAB to check those boarRAB out, I never noticed them before.
 
I had 2 injections today, one for constant low back and buttock pain from facet joint arthritis, and one for a bulging disk that is pinching a nerve, also causing low back pain and cramps in legs, one side more than the other. I believe the pinched nerve area is at L 2-3. Then, before my injections today the PM doc told me that I also have another bulging disk below that one. ( doc did not tell me this ) I got very temporary pain relief only while in the hospital and still feeling the anesthetic from the sedation. Before this, I had constant low back and buttock pain, and cramps in my calves, more on the right side. After standing too long about 5 days ago, my back worsened considerably, and pain shot into my left hip, and then all my symptoms moved to my left leg. After my injection today, my symptoms moved to my right leg, then back to my left, then right, then left. All the pain I had before the injection is back. I've had these injections before for treatment of the herniation years ago, and this is the same thing that happened then too. The ones for the arthritis worked, the ones for my herniation did nothing. I am wondering if it is a herniation, and the doctor is calling it a bulge. Also, I should have gotten atleast some relief, there is none. Does anybody know what treatment would be prescribed next? I am tired of this pain. Just sitting hurts, laying down, anything, only laying in a tub of warm water makes it feel better. Thankyou for any help.
 
Well the trigger points were a huge indicator... when you can press on a point the size of a pin head and send a persons back into convulsions that look like EST. If that doesn't set off alarms in your doctor's head well....

The fact that my back muscles were in a constant state of contraction was another indicator. Also, visual observation of muscle spasms that reserable someone having a seizure but isolated to the back muscles.

Probably the final determinate was my response to the treatments. You ever watch the TV show House? Sometimes they get a diagnosis by seeing how a person responRAB to a specific treatment.

SI's are the same as Basal Branch Nerve Blocks and yes they are a usually used a precursor test before performing RFAs. Typically doctors require having 2 session of BBNB's a month apart and will proceed on with the RFA's if the BBNB's were effective. Though I have heard a couple of stories on this board of doctors performing RFA's even when the BBNB's didn't work.

Myofascial, I am having new problems and searched for this post to re-read your reply to me because I remerabered you mentioning my blood pressure. I had always had low to normal blood pressure, always steady. The low part was normal for me I think because of my hypothyroidism. Awhile back, I noticed one time it was a little high, docs always take it twice now, never had to before, it will be a little high, then a little lower, rarely ever back to what it was. I have posted very recently about my leg symptoms, too much to rewrite, but now wonder if this is all related. I go to doc at 9 AM to get referred to neuro. My symptoms keep changing.
 
Thankyou for your help. I am actually doing better today. I got spoiled by the last few injections I had that helped immediately, but were a dif. area. This time my back was pretty bad, I am not completely well though and still plan to see a neuro. if this continues another week. I have never seen the show " house " but I have heard of trigger points and dealt with a problem with them after injuring my neck in a car accident. I went to PT for about 6 months. Did a neuro diagnose you? And what treatment do you get for this?
 
Actually I was diagnosed by a Pain Management specialist. Anesthesiologist if you were to categorize him more generally. Which makes him very good at Trigger Point injections, Basal Branch Nerve Blocks and Radio Frequency Ablations.

...since then I am back to a point where I am now able to undergo the correct course of physical therapy. Sadly you can't do PT and injections simultaneously since injections require a healing time and PT isn't effective if it isn't consistent.
 
On the discharge paper, it said epidural steroid injection. I am guessing that is what I recevied in both areas. One for the disk, one different area for the very low back.
 
An artery or vein can pinch a nerve just as effectively as an extruded disc or bone growth especially if you have some sort of blockage... an angiograph with contrast can show if you have any blockages of your arteries or veins. Also it would show if you have any collapsed veins ie varicose veins. Which are common as we get older. Veins don't have the benefit of heart pumpage to move the blood so they do so like a series of locks in a canal. Basically they open the valve as pressure builRAB behind them and then collapse back closed when the blood tries to move backward.

Well an steroidal injection is meant to reduce inflammation & swelling... I hate when radiologists provide a vague MRI report. You can have a bulging disc and it can have no relevance or you can have a bulging disc that is impinging (pinching) the nerve against a vertebrae resulting in radiating pain.

In my own personal situation, they found and "extruded" disc at L5-S1. Turns out that it had nothing to do with my pain except that the physical therapy they prescribed for it, exacerbated my condition and "possibly setback my recovery by years" according to the PM specialists that correctly diagnosed "torn/damaged fascia".

Your situation definitely points to a nerve being pinched but was the doctor certain it was a disc pressing on it? It could be a muscle that is out of place that is pressing on it. It could be cardio-vascular in nature. A clogged artery or collapsed (varicose) vein could be bulging and pressing on the nerve. How has your blood pressure been lately? It could be gastro-intestinal related and an organ in the pelvic region pressing against the nerve... All plausible and documented cases.

Personally, I would look for some more referrals and get some opinions from other specialists, since your current diagnosis and treatment isn't providing any relief.

SounRAB like you might want to ask about a SI injection, both sides. I have mine done in a pain clinic. I'm not as lucky as some and have them last for months so I'm moving on to what is called a radiofrequency ablation in May. If this works it could give me as little as a few weeks of pain free living with out pain meRAB or maybe more but if it doesn't we look for something else to try. Also ask you pain doc about Lidoderm patches. I use them every night for the pain in my back. It nurabs the pain. No heavy pain meRAB involved. I have 4 discs out of place, one is bulging. Both SI joints have arthritis.
 
Ty again for your info! I am doing a little better today too and see I was not with it when I first came on here because of the sedative lol! Myofascial, I just started seeing the PM doc where I live and she was recommended by 2 dif people here and so far seems to be very good, she has done all the tests that I believe should have been done on me a long time ago, and checked certain areas on me that with a little pressure brought about pain that I didn't even know were there, which sounRAB similar to trigger points. I will check out your thread.
ReRABilk, ty for your reply. The SI injections are what I get for the arthritis in my low back. I have facet arthritis pain both sides. I had one injection the other day right side, and I had an injection for the bulging disk and pinched nerve in lurabar area, so I had 2, 2 dif areas as they cannot do 2, same area. I will be seeing my PM doc maybe within a week. Those patches sound interesting and better than any meRAB. I will make sure I bring those up to my PM doc, as she listens very well to me. Ty everybody, will give this a little more time, and it looks like I am on the right track with my treatment so far. Going to check out your thread now Myofascial.
 
Thankyou for your information. I do plan to get a referral since the injections seemed to do nothing. The MRI I had was with contrast, and they said one bulging disk is pressing on the nerve. How did they find and diagnose your damaged fascia?
