How hard is it to transfer from a community college to a quality university?

Has anyone gone to community college for around 2 years and then transfered to a quality university?

1. How difficult is this process if you have good grades?
2. Are their scholarship and grant oppourtunities for people who come into school as a transfer?

Any other tips would be helpful.
