How does someone become a Sleep Medicine Physician/Sleep Specialist?

1) Does the Sleep Medicine Fellowship need to be taken after an already specialized residency (psychiatry, pulmonary medicine, or neurology)? For example does one need to become a full-fledged psychiatrist (4 yrs undergraduate, 4 yrs medical school, 4 years residency) before they can take a Sleep Medicine Fellowship?

2) Also does what kind of hours does a Sleep Medicine Physician work? Do they work the same type of hours as a physician in a hospital would and then work with sleep medicine on top of that? Or do they simply work with sleep medicine?
