How does apple fix faulty ipod touches?

do they open the ipod touch and fix it or what do they do when you send it in for repair?
and do they clean it after fixing it?

i'm getting a brand new ipod touch replacement and when i handed in my old ipod touch they said they didnt need the accesories (earpones, usb chord) so when i get my replacement will it come with new ear phones?
