How do you keep fish (plecos and goldfish) alive?

We have had a lot of trouble with keeping then alive, unfortunately. We have a 5 gallon tank and we first bought two fan-tailed goldfish and a pleco. All of them were really small, an inch or 1 1/4 long. The first group didn't live long. The pleco died very mysteriously, and the other two fish got ich and died. The only thing that I know of that could have stressed them out was that one of them accidentally knocked off the filter fixture and got his head stuck there in the tube, but we got him out as soon as possible. And he wasn't the first to die there, so I don't know what the deal was. Later on we got two more fan-tailed goldfish and a second pleco. The second pleco died, but this time we could tell that he was acting a bit weird before he did, and as he didn't live long he was probably sick when we got him. Understandable. Oh, and the fish got more ich... well it can't be easy on you to see your tank mates dying. So we got a snail, thinking maybe the plecos were just not doing well, and the snail died even faster than the plecos. Needless to say we were getting a bit annoyed with this. So we added yet another pleco and the three fish lived together for a long while before the pleco yet again died mysteriously. None of the plecos ever got ich, but both of our goldfish got ich again and one of them died. We have one of them left and obviously enough we are a bit wary about getting any others. He is a black moor, and he's shown a lot of signs of being very healthy so we think he's going to be okay, but we can't figure out what was causing all the problems with the other fish.

We keep the tank at the right temperature and we feed the fish twice a day, as recommended. We washed out the tank after each fish died to make sure that there weren't parasites or bacteria in there that might have contributed that could affect the new fish we put in. We also replaced portions of the water every so often and mixed it up just as recommended, so we don't know what we were doing wrong. Now I have heard that oftentimes fish just die for no reason (and, well, I believe that after seeing what I have! Can anyone give me some idea as to what we might be doing wrong or some tips to help us keep our fish healthier? (We did treat the ich, by the way. Some recovered, some didn't.)
