How do you get a medical profile in the Army Reserves?

I have chronic, moderate-severe knee pain that began in BCT. We were allowed to wear joint support, which lessened the pain. In AIT, we were not allowed to wear joint support without a medical profile. We also were not allowed to graduate on profile. Due to this, I never had the issue treated, as I wanted to graduate on time. Now, six months later, my knee pain has not gone away. I can feel my outer tendon popping when I extend my knees. I have to support myself before sitting down because the pain is so bad. There is also some crunching sounds. The pain often wakes me up at night.

My husband is active duty, and my Primary Care Manager(doctor), assigned by Tricare, is a civilian. I do have an appointment to have my knees looked at.

Since my doctor is a civilian, how would I go about having a medical profile issued, if needed?
