How do you discuss medical needs with potential employers?

I have a good amount of work experience, but several years ago became chronically ill. I am not yet legally defined as disabled, but I know if I want to maintain reasonable health, I am limited in the amount of hours I can work and the kinds of environments I can work in. I am aware employers are not allowed by law to ask about your health condition and cannot take back a job offer after the fact, but how do I go about feeling out a potential job offer without revealing the reasons behind the inquiry? Any suggestions?
