How do I subpoena my ex-wife's bank records?

My ex was ordered by the court to deposit 1/2 of her spousal support into a "travel" account on a condition of her being allowed to move out of state. Since she moved 18 months ago, I have received only 3 statements. I want to subpoena her bank records for the travel account for my order to show cause to cancel the spousal support. As an added note, the support (per the law facilitators office) was not included in the final judgment. And since my ex-wife cannot abide by the court order, I want to have the payment stopped and set up my own account to pay for daughter's flights out for court ordered visitation. I have asked the law facilitator's office what form to use, but they do not have an answer. They are only referring to a lawyer, which I cannot afford, or the local law school. However the law school no longer provides assistance for family court matters. Any help you can give me would be appreciated. I need forms for Sacramento County, California. Thanks!
TO WRG - Read my post please. I did contact the law facilitators office. They are referring me to a lawyer referral service or the local law school. I know for a fact that the law school no longer represents cases of family law, which this is. The only form I can find online in Sac County is for small claims. This is not a small claims case. I do not know where to turn and want my ex to stop lying to the judge, our child and me. Thanks for your "help".
