How do I report my income from tips delivering food for a restaurant?

I get paid cash from my side job. I'm not a dishonest person and I was always planning on reporting it, there's no reason why I should let a small amount of cash hurt me in the long run. Well I am moving on to a new job with the government and I need to show where my income comes from.

How do I file for this? I don't receive a pay check. I get a delivery fee plus tips. So can I file as a 1040 (I know not 1040EZ) or do I file as a 1099 or 1040c or something?

Before anyone answers, not reporting my income is not an option because I'll have to explain where the money was coming from and it'll show I have bad character if i lie about it.

Will I have to pay a penalty? I don't actually make much money at it. It's a side job for me while I'm in grad school and I've just done it on the weekends. I believe it comes up to about 5000 dollars in income over the last 6 months.
