How do I replace the PCV Hose on my 2005 Mazda 6?

My check engine light just came on yesterday and the dealership tested it and said my PCV hose was cracked and they want to replace it and the gaskets. The want to charge 500 for parts and labor and I'm thinking I don't want to go that route. Where is this located and is it easy to fix?
 
$500.00 dollars? all you need is a piece of 3/8 hose. just hook 1 end of the hose to the intake and the other end to the pcv valve.
