How do I read a knitting yarn label?

I am not sure how to read this yarn label or how to figure out what needles to use or how many stitches per inch (I'm making a hat so my stitch amount has to be pretty accurate if it will fit). Can you tell me what each of these label components mean?:

1) Gauge for 4" x 4" (10cm x 10cm)

2) A square with a pictures of knitting needles inside (indicating it is info. for knitting not crocheting) with the the numbers "13" 9mm, 12 R, and 9 STS around the sides of the square [This is especially confusing for me]

3) Super Bulky weight

I have read some knitting websites but their explanations don't make sense to me. I'm hoping your explanations will help me understand!
