How do I properly format my 4G SD card?

I've been using a 4G SD card to transfer files between my Macbook Pro and my old Dell PC. Somehow I've messed up the formatting because suddenly my PC wouldn't open it without reformatting and when I try to format it recognizes it as only a 200MB card.

How do I fix this?
My Dell PC is an Inspiron 1525 laptop running Vista.
