How do I learn to talk in front of an audience?

I used to be really, really shy and have no confidence but I went to counselling and I got a million times better. However I still find public speaking really, really tough. I can really feel my heart beating in my chest, my voice shakes, I can't think, I can't read what I'm meant to be reading and I just can't get my words out-it feels out of my control.
When it gets to an audience of about 3 people I start to feel more stressed but it's manageable. But much more than that I find myself desperately struggling. Today I was meant to be presenting in front of an audience of about 200 people and I screwed up so badly it was humiliating! It was like the speech at the beginning of she Kings Speech but without the excuse of a stutter!
Any ideas of what I can do to overcome this? I'm desperate! I'm so much better at 1-to-1 conversation now and it depresses me that I still have this weakness-It makes me feel like I haven't progressed at all!
