How do I go about sending a thank you note to my doctor?

I went to the family doctor for a checkup recently. It really wasn't a big thing at all and everything came back fine. But I hadn't had a checkup in a couple of years, so I was sort of dreading it, and also I wasn't exactly in the best shape (college will do that to ya). But this doctor really made me feel comfortable and sort of alerted me about eating healthier without being patronizing or anything. Good doctor, basically. And since my checkup I've been working out regularly and eating healthier. I feel really good.

Anyway, I've been thinking about sending off a thank you note, but I'm not really sure what to say. "Hey Doc, Thanks for doing your job!" just seems useless, but that's really all there was to it... and yet, I do feel compelled to show my appreciation just because it was that visit that really spurred me on to living better.

Any tips for me? Or, should I just not send a note at all?

Also, if I were to send the note off, do I address it to the hospital where the doctor works or is there something in the hospital or what?
