How do I get clean, clear, crisp photos?

I have a Nikon D90 with the lens it came with. I have one other lens 50 200mm. I know it has to do with lighting and settings. But i cant for the life of me get my pictures to look crisp and clean. It just seem like the harder I try the more steps I go back. I just want to know if there's a book I should be reading a website. Just something to point me in the right way. Thanks for all your help.
 
Take a deep breath. Relax. Now find a comfortable seat and sit down. Begin READING & STUDYING the Owner's Manual for your camera. By doing this you will actually begin to learn how to really use your camera.
