how do i get a no running profile for the army national guard?

if i just got diagnosed with asthma, can i get a no running profile for my pt tests because the running just kills me? if so then how do i go about doing it and can i get out of running this week even if i dont have a profile yet? do i need a note from a doctor saying that i cant run or something?
