How do I fight a bogus collection bill?

Hello and thnaks for your help.
My wife joined Bally's Total Fittness, a couple of years ago with a 2 year contract. A few months later, she had some medical conditions that her physician stated would prevent her from working out. The Bally's contract clearly states that you are released if you obtain a physicians note due to a medical condition that prevents you from using their facilities.
We go there and try to hand it to them, they refuse and tell us it needs to be faxed. We fax it to them, they still bill us. We call and explain the situation and after weeks of arguing, they make us get another doctors note. We get a new doctors note and fax it to them.
They then go and sell our account to a collection agency. We are non the phone with them for months. We faxed all the paperwork numerous times and they finally agreed and closed our file.
Last week we get a call from another collection agency stating that they bought our debt and we owe them $700. I am tired of this, is their any recourse I can take? Please help.
