how do i convert a php form to hebrew?

i have this php submission form originally written in english. how should i convert it in order for it to work in hebrew?

i am not looking to translate a webpage that i see. i am building a php form submission for the web. i need to translate and convert the english version into hebrew.
i have translated it already but the data is sending jibrish. i need it to understand that it is hebrew.
