How do I change this HTML code?

Alright so my friend sends me this HTML code and wants it to not send the entered information to the website so the people can login but yet to his email. I will post the HTML link at the end but allow me to explane what I need your help with. I have tried and tried to change it but keep finding new problems so I need someone that knows a little more then me to give me a hand with this. I will tell my friend that you helped me and even give them your name and email so they can contact you and you may also get paid for some jobs he needs done.
