How do I add an image header under the description on my Tumblr?

The theme I have only allows one or the other. I added a CSS that would add the image header, but only added it and removed the title/description. I want to keep the description and my title but I want an image underneath. Custom HTML is allowed with the theme. How do I add the image header underneath?
