how can I make a web page (fill-up form) that sends a word file into an email?

here how it goes,

i need to make a web page that can be accessed by specific people, its an online request form that they will fill up. when it is submitted an email will be sent to the admin containing a form that is already filled up.. or an attachment (.doc or .xls file) that can be printed..
