honda idle problem when i start the car it will idle up and down like im reving it

up or something? it start when i start the car but when fan is on in the car for the heating and cooling it wont do it so i have to either blast the heat or ac to get it to idle right. it just keep jump up and down like
vooooom..... voooom..... voooom til i put the heat or ac on full blast
not sure what it could be so any help would be greatful thank you
