Honda Civic Clutch Won't Disengage?

I had a sudden clutch failure yesterday. I haven't had a chance to look too closely at it yet. I wonder whether some of you mechanics could mention the possible and the likely points of failure based on the symptoms.

This 2001 Civic has 100K miles since the last clutch job. It's not warn out. The clutch holds tight. Everything was fine when I pulled onto the freeway yesterday. But when I got off the freeway, the pedal was twice as heavy as usual, and the clutch didn't disengage at all.

I coasted into a parking lot and shut the engine off. Ran through the gears a few times. Tried working the pedal a number of times. No change. The clutch is still firmly engaged with the pedal to the floor. And the pedal is still heavier than usual.

About a month ago, I recall hearing a little noise from the throwout bearing. Just a slight scratchiness while the clutch was in. After an hour, the noise went away and never came back. Possibly related?

So the thing I'm wondering is . . . Can this be a problem with the pedal or hydraulic system? Outside the trans-axle? Or is the problem inside the bell housing?

I know! You can't tell based on a thin customer description. And the only way to know is to get in there and look at it. But still. Based on what you've seen, what would you think is most likely?
