help with oscar fish skin disorder?

my oscar about 2 years old is losing scales and has hole in the head disease. i tried mellafix and ive also done frequent water changes but nothing seems to work. what medicines are available or what could i do to get him healthy again?
 
hole in the head is bad and pretty hard to cure. cured it ONCE with Jungle Labs Hole-N-Head Guard. No other medication has worked for me. Also clean u water really good before adding more medication. i hate to say it but the best cure is a fresh start. But two years is a long time, i lost mine at 3 years and it sucks.
