help with filling out a taco bell application?

for my taco bell application this would be my first real job i need help with the following things:
employment interest and the starting wage/salary desired?? idk what to really put
what kinda references??
and for emplyment record what do i put because i dont really have one cause ive never worked before but i dont want to have a clown looking application
