A clinic operates five branches within a country.
At any consultation the nurse is expected to collect information for a patient under the following headings:

Age
Name of Guardian
The doctor then attends to this patient collecting information on vital signs as follows

The consultation then takes effect to know what the patient is sick of , as follows:
