Health BMI test on the internet?

Well a few years ago my sister and i did a BMI test online it was more than just your height and weight and age though. there was all questions about your diet, lifestyle, excersize etc. I really want to take this test or one like it so please any help would be greatly appreciated! xx
Yes i know that they arent accurate i just wanted to know if any one knew the test and what its callled im not taking it that seriously
