Grandparent dying? How to talk to my kids?

My grandma is in the hospital right now with serious heart problems. She is going to have to have bypass surgery, but her doctor is giving her a 50/50 chance she will make it out alive.

I have a 7 year old and a 3 year old, and my question is: How do I tell them that Grandma is sick? Is there any websites I can go to?
