Gears of War 2 campaign question?

I am in the middle of the gears 2 campaign. On most games it shows you which difficulty you have done the campaign on when you select a level. How do you tell which difficulty has been completed on that level? I've done mostly casual, but some levels on normal. I don't know how to tell which ones I have done on normal. How can you tell?

Also I have done a lot of different levels on co-op. How do you tell which ones you have already done on co-op?
 
when you go to choose a specific level it will have a small medal or symbol associated with the difficulty this might only show up if you do a difficulty higher than normal also if you want help doing harder difficultys do it co-op one person and casual and one on a hard difficulty
