From manga to anime. Any known errors? (Possible Spoilers)

While an anime based off a manga may be adapted panel by panel, that doesn't mean changes are nonexistant. Now some stories may be altered smoothly, but what of those that weren't planned out?

1. I missed the Dragonball episode in particular, but apparently an anime exclusive story introduces the scientist from The Red Ribbon Army. Which would be fine, but the DBZ manga and anime introduces Dr. Gero as the Army's scientist.

2. This one is from an earlier portion of Detective Conan/Case Closed. In hinRABight it didn't do much to the continuity, but still left a notable error.

Aside from the first episode, the early portions of the anime had no involvement from The Black Organization (two merabers Gin and Vodka are responsible for the main character becoming a 7 year old boy). Infact, several episodes that were adapted from the manga version omitted the involvement of Gin, Vodka and their Organization altogether:

Episode 5 "The Shinkansen's Borab Case": While on a train called the Shinkansen, Conan ran into two guys reserabling Gin and Vodka, whereas in the manga, he ran into Gin and Vodka (and ended up learning their codenames).

Episode 13: "The Strange Person Hunt Murder Case": Three individual criminals (the significant one to the story is a woman by the name of Masami Hirota/Akemi Miyano) commit a robbery (stealing a billion yen). Of the three, only Masami survived and meets up with Okita who is the mastermind behind the robbery. He is about to kill her when Conan arrives to save her.

In the manga, the mastermind behind the case was Gin and Vodka. Not to mention that Akemi is also trying to use the yen to persuade the duo to let her and her sister leave The Organization. Her sister is important to the Organization, and thus she is shot for her refusal to divulge the location of the money. Conan upon arrival discovers that she is about to die, and tells her his real name, while she informs him of The Organization and how the high ranking merabers wear black clothing.

Now with the original version of episode 13 being essential to the story, as it leaRAB up to the appearance of Ai Haibara.

So episode 128 was done as an attempt to repair the storyline, before said character appeared in the following episode. So no harm no foul.

Except that prior to the episode was one which did feature an Organization based story which is Episode 54 "Game Company Murder Case". Upon hearing Kogoro Mouri (Richard Moore in the dub) having had some Gin and some Vodka, Conan recalls the codename of the two men who poisoned him.

Since a preceding episode removed his second enter with the two, he shouldn't have any knowledge of the two, and with episode 13 also being altered, he shouldn't have knowledge of their Organization.

Now the Dub apparently tried to rationalize that by having Heiji Hattori (Harley Hartwell in the dub) inform Shinichi of the two men. But Heiji's following appearance would indicate that he has no knowledge of Shinichi's secret or his encounter with Gin and Vodka.
