In 08, my employment condition was very complicated; I was employed from January to early October. In between, I received 2 months of U.I for family medical leave. Afterward, I was hired as a rehab coach, not as a staff, but as a sub-contractor (thus self employed???)

Because I was a sub-contractor from October to December, I did not receive any T4 from my boss, and I was told by my boss to "create my own company" in order to file a fax return??? I earned $1000 as self employed person last year.

Would you please explain to me how am I going to "create my own company"? What professional advices you would be willing to offer me, so that I may have maximum tax return?? If I walk in to your office as my employment condition stated above, how much would you charge me? Please help me, as currently, I am financially tight.

Thanks in advance.
I am currently working for an auto related accident rehabilitation.
