For anybody whos really good with HTML...?

I have an assignment due for my Intro to Computing class and I'm having trouble on a couple of things. I am really struggling with how to set up an audio and video feed on my website. I have been following w3schools.com but I cant seem to get my file to play.
 
Post your code. Make sure you have all of the basics right like tags closed and spelled right and things like that. It always seems to be the little things that are done wrong that screw up the whole thing.
