Flash and PHP form submission for multiple fields?

I want to create a form in flash cs3 used actionscript 2.0 to send an email upon form completion. I already have the destination email address, the subject and the fields i want to include. My question is how do i make the php form to send multiple fields in a single message. Like... name, age, phone number and more all in the same message
