I've been getting EMA 2 years now. I filled in the first really long form where there is a part a & b, one to be filled in by you and one by parent(s). Then I sent that off and they sent me a slip of paper back that I had to take into college to be signed by my tutor on my first day. The college then send that back to EMA for you. I think the second one might just be to prove you actually did go to the course you enrolled for or something, I don't really know the reason.
