Doctors note for swim?

Well i want to know if you think my doctor will give me a note to get out of swim. Whenever I'm in or close to water i get massive headaches. I don't know why but i do. But i don't want to go to the doctors if i won't be able to get a note. But if i can't oh well i will just get over it and do my best
