Doctors, Defense Lawyers, and Morals?

So, in my way of thinking, being a defense lawyer or a doctor would be terrible even though the pay would be nice because if you are a defense lawyer, you might have to represent someone who is truly guilty and thus you allow them to remain free and possibly commit the same crime (maybe murder) again. Likewise, if you are a doctor, you have to sacrifice your connection with people because otherwise you would never be able to make a decision lest your patient suffer severe problems from it. So my question is, how do defense lawyers and doctors deal with the moral conflicts?
