Appeal Letter for Reinstatement for Financial Aid
I am writing this appeal with the hopes of gaining my financial assistance back for educational purposes. While attending Grambling in the spring of 2009, I was treated for ADHD, by Dr. M G Mickail, located at Northeast Health Unit, in Monroe Louisiana. Dr. Mickail prescribed a medication by the name of Strattera, which I used to help control the disorder of ADHD. While taking this medication, I began having trouble sleeping at night, which caused me to sleep during the day that in turn caused me to miss my scheduled courses. We did not figure out the cause of this sleeping disorder until the end of my classes. I have attached information regarding the definition of this disorder, side effects associated with it, and also the prescription that I was prescribed to control this. In order to rectify this problem and assure that this does not happen again, I have scheduled another appointment with a different provider to prescribe a lower dose of medication, that will also help me control my disorder, while staying awake to deal with the day to day activities of school and life. I am requesting that this office give me another chance at my academic adventures at Grambling State University. I feel that I should not be controlled by my disorder, but find a way to control it, and achieve the goals that I have set forth for myself.
