at some points, yes. Especially around the Cuban Missile Crisis. At most other points neither side was really willing to use force against the other in my opinion, out of fear of Mutually Assured Destruction (MAD).
 
there's something called the doomsday clock which tells how close we are to the end considering all the variables. We are closer to midnight now than then.

or actually not, now it's 5 min to midnight. in 1984 it was 3 to midnight under raygun and 4 to midn under carter in 81, when he didn't go to moscow olympics and cut talks with moscow.

There have been times when the powers of the world neared major world war which likely would have included nuclear arms being used.

It is possible now that some terrorist group or run away radical government may use some weapons of mass destruction, however, that doesn't translate into the end of the world.

It has been possible in the past things could have happen that unbelievable numbers of people could have been killed and/or injured but again that doesn't equate to the end of the world
