Do fish breathe the Oxygen from the H2"O" or from the Oxygen dissolved in water?

When fish breathe, do they take the oxygen directly from the water molecules, leaving H2, or do they take the oxygen which has dissolved in the water? Also, how does water dissolve in the water, does it have to be moving?

I'm asking because I want to make kind of like a pond, so I want to learn about how it all works... So I can understand about the pumps and filters...
