Do Chicago-based freelancers need to register as a business (for city, state or both)?

I have been doing some freelance work and want to stay on the level for tax purposes and such. Please only answer if you know the rules for Chicago and/or Illinois freelancers. Even with what I am able to Google on the Web, I find learning the rules on all this stuff very confusing and overwhelming.
