Dealer Disclaimers (Input please)

I was wondering what you guys have out there for Disclaimers at your guy's shop regarding flashing/unlocking/repairing phones. im making one right now and i just want to see what you all have and or ideas, i got pretty much one down its general and to the point.

I used to have one but stopped giving it out. Here lately I just tell people with water damaged razr's that there's a chance that the flash will fail and their phone will be a paper weight.

I probably should start putting some out though..

I'd like to know what other dealers have as well.
 
I have those too but like i was talking abou one where it talks about flashing the phone and how if something goes wrong its not your fault etc and same with repair, i just got done with mine but i just wanted to make sure i coverd my bases i guess
 
i have another one i can post in the morning. honestly, i make people sign these forms b4 i do their phone. i dont brick phones often, but really dont like paying out when there is a janky phone that goes bad
 
I never tell them anything unless they look like they would ***** but then I just give them something similiar from used cellphone pile plus I only pay 3-5 dollors most used phones unless they have sumthing good anyways which most dont

on the qpay yes but I ve seen a customer bring in a activation form/ reciept/ with his info but I ve asked if I were it was from and he said he got at wireless toyz
 
Mine states this,
Pre-payment is required for (re-programming, unlocking, flashing, repairing) your cellular or computer device
 
i use quickbooks, and there are multiple differnet templates that youcould create that would make receipts like this. make a form and have it input ptn, name, item, esn, yada yada....would be failry simple
 
on a pos system, for one that prints on reciepts, i have it set up, so i have 2 dif discalaimers, mandatory one stating. that with basic flash, only talk and text will work, and if the bring it back within a certain time frame, it will just cost difference, 2 full flash if possible.

then a selectable one, for wtr dmged fones, that not responsible for software problems (bricking) due 2 the board being wet at one point or another. and a very slight change, with full flashing, that it will ruin fone.

aww, much better thanks joey :P i kept clearing cookies, but anytime i went bak 2 hofo, was still on tht name :P was a old employees. and i did not want 2 use it rofl :P
