Cooks' secrets for perfect chicken soup?

I made chicken soup tonight, but the chicken ended up kind of tough. I usually just throw a raw breast of chicken into the pot, along with veggies, noodles and broth. The only thing that I did different this time is wait until the broth was boiling before I put the chicken in. Is that why it ended up tough?
Are there other secrets to perfect chicken in chicken soup?
