Computer having annoying problems with media.?

An older PC - Pentium 4, 2.8 Gghz, 2 gig of ram, Sound Blaster Audugy sound card, and an Invidia Geforce graphics card.

The computer was well above the minimum, so I installed Vista Ultimate on it. But soon after I started having problems with any form of media under certain conditions.

The condition is, after having the PC on for a while, WMP and the like will begin to lag and skip on audio and video. Streaming video on my web browser will also begin doing this, at least while the video is still loading (it doesn't do this at first). It will especially kick into this annoying habit if the PC goes into auto-sleep, and I have to wake it up. Usually, the only way to fix it (for a while), is to restart the computer.

What is wrong? How can I fix it? I'm desperate here... someone, anyone, please help. I have confidence in your brilliant minds!
